Position Overview

The Case Manager works collaboratively and effectively conducting Case Management assessments. Develop and monitor Individual Service Plans with measurable goals.

Essential functions:

  • Manage a caseload of clients by conducting Case Management Assessments and documenting information in the electronic health record.
  • Assess, plan, implement, monitor and evaluate services needed.
  • Make relevant referrals with appropriate guidance and follow-up.
  • Maintain daily case notes and enter information into database in a timely manner.
  • Acts in accordance with related Social Work regulations, laws, Counseling and Case Management services, including HIPAA; Duty to Report, Client's Rights.
  • Update all records to meet CARF standards.
  • Other duties as assigned.
Knowledge, Skills and Abilities:
  • Effective written and oral communication skills.
  • Ability to articulate client needs through the assessment.
  • Ability to work well under pressure.
  • Able to work well with visually impaired individuals, including the elderly.
  • Knowledge of community resources.
  • Ability to develop individual service plans.
  • Proficient ability to collect appropriate data, analyze and draw valid conclusions.
  • Ability to write DARP case notes.
  • Proficient use of computer applications including Microsoft Suite and the EHR.
  • Knowledge of Social Work regulations, laws, Counseling and Case Management services, including HIPAA, etc.
  • Creative and flexible problem-solving skills.
  • Ability to accept and act on feedback.
  • Knowledge of mental illness, substance abuse and recovery are a plus.
  • Ability to perform in a collaborative, team environment and independently.
  • Knowledge of the Rehabilitation Act and the Americans with Disabilities Act
  • Knowledge of common eye conditions.
Qualifications:
  • Minimum six (6) months experience in Social Work, Counseling, and or Case Management.
  • Experience with Blind/Vision Impairment Rehabilitation preferred.
  • Bilingual (Spanish and English) a plus.
Education:
  • Bachelor's Degree in Social Work or related field required.
  • Candidates actively working toward Social Work licensure in the State of Ohio will be considered.
  • LSW or LISW credential is preferred.
