The Case Manager works collaboratively and effectively conducting Case Management assessments. Develop and monitor Individual Service Plans with measurable goals.
Essential functions:
Manage a caseload of clients by conducting Case Management Assessments and documenting information in the electronic health record.
Assess, plan, implement, monitor and evaluate services needed.
Make relevant referrals with appropriate guidance and follow-up.
Maintain daily case notes and enter information into database in a timely manner.
Acts in accordance with related Social Work regulations, laws, Counseling and Case Management services, including HIPAA; Duty to Report, Client's Rights.
Update all records to meet CARF standards.
Other duties as assigned.
Knowledge, Skills and Abilities:
Effective written and oral communication skills.
Ability to articulate client needs through the assessment.
Ability to work well under pressure.
Able to work well with visually impaired individuals, including the elderly.
Knowledge of community resources.
Ability to develop individual service plans.
Proficient ability to collect appropriate data, analyze and draw valid conclusions.
Ability to write DARP case notes.
Proficient use of computer applications including Microsoft Suite and the EHR.
Knowledge of Social Work regulations, laws, Counseling and Case Management services, including HIPAA, etc.
Creative and flexible problem-solving skills.
Ability to accept and act on feedback.
Knowledge of mental illness, substance abuse and recovery are a plus.
Ability to perform in a collaborative, team environment and independently.
Knowledge of the Rehabilitation Act and the Americans with Disabilities Act
Knowledge of common eye conditions.
Qualifications:
Minimum six (6) months experience in Social Work, Counseling, and or Case Management.
Experience with Blind/Vision Impairment Rehabilitation preferred.
Bilingual (Spanish and English) a plus.
Education:
Bachelor's Degree in Social Work or related field required.
Candidates actively working toward Social Work licensure in the State of Ohio will be considered.
LSW or LISW credential is preferred.
