What You Will Do - Essential Functions

The Medical Assistant reports to the Director of Clinical Administration and supports the Chronic Care Management Program by providing both clinical and administrative assistance to the care delivery team. This role is integral to ensuring coordinated, high-quality, and cost-effective patient care by supporting care teams, facilitating communication, managing documentation, and assisting with care transitions across healthcare settings.
  • Patient Care Coordination & Communication
    • Conduct pre-visit intake and confirmation calls, gathering medical, social, and medication histories.
    • Communicate lab results, imaging findings, and care plans to patients after provider review.
    • Coordinate scheduling for in-person and telehealth visits, including rescheduling and follow-up on missed appointments.
    • Serve as a liaison between patients, providers, case managers, facilities, and specialists to support continuity of care
  • Care Transitions, Tracking & Documentation

    • Track patient admissions, discharges, and activity across hospitals, skilled nursing facilities, and other care settings.
    • Document care transitions in the electronic medical record and notify appropriate care team members.
    • Obtain, request, and upload medical records to support post-discharge and ongoing care.
    • Assist with coordinating respite stays, long-term care stays, and return-to-residence visits.
  • Clinical & Administrative Support

    • Provide operational and clinical support to primary care market teams.
    • Process medication prior authorizations, prescription refills, and insurance-related documentation.
    • Manage the clinical inbox and escalate clinical concerns to RNs or providers as appropriate.
    • Support clinical quality improvement initiatives and participate in multidisciplinary team meetings.
  • Compliance & Professional Standards

    • Adhere to organizational policies, ethical standards, and compliance requirements.
    • Maintain confidentiality and security of protected health information in accordance with federal, state, and local regulations.
Who You Are

Required Qualifications:
  • High School Diploma or equivalent
  • Completion of an accredited Medical Assistant training program.
  • Basic Life Support (BLS) certification
  • 1+ years of experience in a clinical, healthcare, primary care, or outpatient clinical setting.
  • Familiarity with EMR systems and basic clinical documentation.
  • Proficiency with Microsoft Office Suite, including Outlook, Word, Excel, and Teams.
  • Customer Service experience
  • Ability to pass background check upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/trace search (LEIE), education report (highest degree obtained), civil upper and lower search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen.

Working Conditions:
  • High-speed internet service (cable or fiber optic) with minimum download Speed of 20 Mbps, Upload Speed of 5 Mbps, and Maximum Latency of 100 milliseconds (must be installed before starting) required.
  • Frequent use of computer and phone systems
  • Must be able to constantly remain in a stationary, sitting position, communicate and exchange information with others, inspect information, perform repetitive motions with arms and fingers, interpret data, problem solve, make decisions, organize and plan, and maintain a positive and professional attitude in all situations.
  • Work is performed from home with company-provided equipment. Sitting for long periods of time is expected and use of fingers and hands for typing is necessary.
  • A quiet workspace with minimal background noise for calls.
