Position Overview:

We are currently seeking a compassionate and motivated Social Worker 2 to join a primary care clinical team. The successful candidate will work closely with healthcare providers to support patients' social needs, promote care continuity, and coordinate access to community resources.
Key Responsibilities:

Social Support & Community Resources:

• Address social determinants of health (SDOH) including housing, food insecurity, and transportation.
• Conduct social needs assessments and refer patients to appropriate community resources.
• Maintain and update a resource list of local services and eligibility criteria.
• Collaborate with other social workers and healthcare team members.

• Assist with hospital discharge planning and patient follow-up.
• Help obtain patient records and medical equipment as needed.
• Monitor care plans and re-engage patients lost to follow-up.

• Facilitate specialist and imaging referrals.
• Track appointments and follow up on reports.
• Maintain updated referral contacts and cost/resource data.

• Guide patients in accessing affordable medication options.
• Support applications for programs like CoverRx and RxOutreach.
• Liaise between providers and drug manufacturers when needed.

• Document all patient interactions and follow-ups.
• Provide health education and connect patients to appropriate learning tools.
• Participate in staff meetings, community health councils, and onboarding sessions.

• Computer, phone, fax, scanner, printer, copier, calculator
• Personal vehicle (for occasional travel if required)

• Bachelor's or Master's degree in Social Work or a related field
• Strong knowledge of community resources and patient navigation
• Effective communication, organization, and problem-solving skills
• Previous experience in healthcare or social services preferred
• Ability to work independently and as part of a care team
