The Clinical Coordinator of Care is responsible for patient care in their own discipline, facilitating and developing the relationship/coordination between the facility and rehab. This includes all aspects of the clinical practice focusing on optimization of documentation, appropriate functional outcomes, identification of training needs, and implementation of medical record reviews. Objectives will be achieved within the value system of Broad River Rehab and with the ultimate objective of creating value for our patients, our customers, and our employees.
